Esketamine (Spravato)

Kela-reimbursed — strictest in Europe

Reimbursed since August 2023 under two restricted entitlements (codes 3062 and 1539, the latter 100%), with some of the strictest criteria in Europe: TRD on an SSRI/SNRI, MADRS ≥31, at least three failed treatments with attached psychotherapy (one a combination), documented in a psychiatrist’s B-statement. Unique delivery model: you buy the reimbursed spray at a pharmacy and take it under supervision at a healthcare unit. Entitlement is time-limited to one year; continuation requires MADRS scores to have at least halved.

Ketamine Therapy

Hospital only, occasional

Occasional off-label psychiatric use in hospital settings; no established private infusion market.

Psilocybin

Trials only — none recruiting

Research-only, and no Finnish psychedelic trial is recruiting as of July 2026.

MDMA

Trials only — none exist

Research-only in principle; no Finnish MDMA trial exists.

Medical Cannabis

Highly restrictive

Sativex (authorized 2012, MS) plus case-by-case Fimea special permits (erityislupa) for anything else after conventional options are exhausted — under 100 active unlicensed-product patients; no reimbursement. Recreational: strictly illegal (use itself punishable).
