GKV-reimbursed for treatment-resistant depression (≥2 failed antidepressants in the current moderate-to-severe episode). A nationwide specialized program excludes Spravato from standard prescriber budgets to ensure solid patient access.

Off-label infusions at private practices in most large cities, roughly €200-400 per session. Note that depression guidelines recommend administration under clinical supervision with an observation period.

BtMG Schedule I. Strictly illegal outside of registered clinical trials. However, Germany has active pre-approval compassionate-use trials running at selected psychiatric hubs: psilocybin for depression PEX010 license is valid until 17 June 2027.
